Bulbapedia’s release note reads: “This card was included as both a Regular card and a Secret card in the Flashfire expansion, first released in the Japanese Wild Blaze expansion.” It adds that the Regular print was reprinted as a Mirror Reverse Holofoil in the Japanese Premium Champion Pack. The database text for the attack: flip a coin until you get tails, and add 30 damage for each heads. The special rules printed on the card say that a turn ends when this Pokémon becomes a Mega Evolution Pokémon, and that when a Pokémon-EX is knocked out, the opponent takes 2 Prize cards. Per Bulbapedia the UR carries the same game text, so what separates it from the RR is the number and the rarity code.
The Set: Wild Blaze (XY2)
The official product page names the set ポケモンカードゲームXY 拡張パック「ワイルドブレイズ」 (Wild Blaze), released on Saturday, March 15, 2014 (2014年3月15日(土)), at ¥150 plus tax per pack with 5 kaarten per pack. The page prints no packs-per-box figure and no English pairing.

The database holds two rows for MガルーラEX. Adding the UR print from the Bulbapedia page gives the three Japanese prints the archived page describes: the Wild Blaze RR, the Wild Blaze UR and the Premium Champion Pack reprint. The Champion Pack row is the one the database shows under CP4 094 / 131; Limitless calls that product “EX x MEGA x BREAK Champion Pack” and lists the print as #94.
